Social Work Intern

POSITION PURPOSE

• Shadows and works to assess patient's medical condition, psychosocial concerns, home environment and additional information to provide appropriate discharge placement. Observes and learns communication styles with family and/or significant other, staff and outside agencies to discuss discharge-planning alternatives. Assists patients/families in coping skills and provides emotional support. Learns consultation reports and documents activity in patient medical record. Performs other related duties as needed.

MINIMUM EDUCATION REQUIRED

• Progressing in an accredited social work program.

MINIMUM EXPERIENCE REQUIRED

• Introduction to social work class is in progress.

KNOWLEDGE, SKILLS & ABILITIES REQUIRED

• Experience in the health care arena preferred.
• Must be able to read, write, speak, hear and comprehend the English language.

PRINCIPLE ACCOUNTABILITIES AND ESSENTIAL DUTIES

1. Developing skills to complete a patient’s medical and psychosocial assessment for discharge planning.
2. Learning proper communication skills with multidisciplinary team.
3. Documents assessments in the patient’s medical record under supervision.
4. Assists patients/families in coping skills and provides emotional support
5. Developing knowledge of and ability to discuss financial alternatives with patients and families.
6. Observes and participates in interdisciplinary meetings.
